There are a few reasons this could happen, but the most common cause is that the domestic hot water recirculation (DHWR) system is not correctly balanced.

DHWR systems need to be balanced to ensure that hot water is available throughout the building on every floor, at every fixture, and at all times. This is complicated because water flows in the path of least resistance and in open systems like these that can change constantly.

Traditionally, ball valves would be installed at the end of each branch in the system and manually adjusted to a set flow rate. This process takes a long time and is often not very accurate in a dynamic system with fluctuating pressures.
